• Entries must be from legitimate healthcare organizations
• Achievements must be within last 24 months
• All information must be true and verifiable
• One entry per category
• Entry fee: $399

Your Submission

• Must be in English
• Must include required elements
• Must be your original work
• Cannot contain confidential patient information
• Can be edited until submission deadline

Recognition Process

• All entries reviewed for eligibility
• Decisions are final
• Winners announced on [Date]
• Winners can purchase recognition packages
• No refunds after submission

Using Our Brand

• Don't use our logo without permission
• Don't claim to be a winner before announcement
• Winners must follow logo usage guidelines
• Don't modify our branding

Privacy & Data

• We protect your information
• We may share winner details publicly
• We may use entries for promotion
• You keep ownership of your content

Important Notes

• We can change these terms
• We can reject entries (in which case a full refund is issued)
• We can revoke recognition if terms are violated
